ABSTRACT

Introduction: The school population represents a fundamental group for health promotion actions, given that the foundations of healthy behaviors in adult life are established in children and adolescents, who can be greatly influenced by the school. Objective: To describe the health-related behavioral factors of schoolchildren between 13 and 17 years of age in secondary and middle school in Colombia. Methods: A national cross-sectional study was conducted (i.e., School Health Survey -ENSE-) that recorded information on dietary practices, physical activity, alcohol and drug consumption, injuries and bullying, and oral, visual and hearing health. Sampling was probabilistic, cluster and multistage. Results: Schoolchildren have low consumption of fruits, vegetables (13.1%) and dairy (76.5 %), high consumption of ultra-processed foods (82.4 %), sugary drinks (74.0 %) and fast foods (14.8 %), frequent addition of salt at the table (43.3 %), low compliance with physical activity recommendations (15.0 %) and high sedentary lifestyle (46.3 %), frequent consumption of alcohol (44.7 %) and psychoactive substances (14.7 %), in addition to prevalent situations of bullying (15.4 %), rejection (8.2 %) and verbal aggression (42.7 %). Conclusions: The ENSE shows critical inequalities by gender, ethnicity and social class, throughout the country. The indicators observed in schoolchildren are precursors of various chronic and degenerative diseases and mental illness, which requires the urgent attention of the different social actors in the country.


Introducción: La población escolar es un grupo fundamental para las intervenciones de promoción de la salud, ya que en los niños y adolescentes se sientan las bases de un comportamiento saludable en la vida adulta, que puede ser muy influenciado por la escuela. Objetivo: Describir los factores comportamentales relacionados con la salud de los escolares de 13 a 17 años de edad que cursan educación básica secundaria y media en Colombia. Métodos: Se realizó una encuesta nacional transversal (Encuesta de Salud en Escolares -ENSE-) con los lineamientos de Global School-based Student Health Survey para recolectar información sobre prácticas alimentarias, actividad física, consumo de alcohol y drogas, lesiones e intimidación, y salud bucal, visual y auditiva. El muestreo fue probabilístico, por conglomerados y polietápico. Resultados: Se incluyeron 79.640 escolares de 298 municipios. Los escolares presentaron bajo consumo de frutas, verduras (13.1%) y productos lácteos (76.5 %), alto consumo de alimentos ultraprocesados (82.4 %), bebidas azucaradas (74.0 %) y comidas rápidas (14.8 %); alto uso de sal añadida en la mesa (43.3 %), actividad física inferior a la recomendada (15.0 %) y sedentarismo (46.3 %). Fue frecuente el consumo de alcohol (44.7 %) y sustancias psicoactivas (14.7 %) y reportaron situaciones de intimidación (15.4 %), rechazo (8.2 %) y agresiones verbales (42.7 %). Conclusiones: La ENSE muestra desigualdades por género, etnia y clase social en todo el país. Los hallazgos observados en los escolares son factores de riesgo de enfermedades crónico-degenerativas y mentales, y requieren la atención urgente de los actores sociales del país.

ABSTRACT

Introduction: As reported by WHO, hypertension is the third ‘killer’ disease, accounting for one in every eight deaths worldwide. Many modifiable and non-modifiable risk factors are found to be associated with the disease. The present study aims to determine the role of dietary management and physical activity in the control of hypertension in a given population. Material and Methods: A cross-sectional study was conducted that. Study included all hypertensive patients (290) from the field practice area of a tertiary care institute. A self-constructed, pretested questionnaire was used to enquire about the socio- demographic factors and other risk factors associated with hypertension. The data was analyzed using SPSS 23 and valid conclusions were drawn by using the chi-square test. Results: Out of 290 patients, 204 (70.3%) were females and 86 (29.7%) were males. Univariate analysis identified decreased salt intake, decreased fatty food consumption, physical activity, stress management, and spiritual help as highly significant factors affecting hypertension. On multivariate analysis, physical activity, stress management and spiritual help were found to be significant. Conclusion: The study showed that physical activity, stress management and spiritual help were significantly associated with hypertension control among the elderly. Measures should be taken to create awareness about lifestyle modifications in the control of hypertension.

ABSTRACT

Objective:To investigate the effects of different light sources on the stability of cinepazide maleate in sodium chloride solution, providing a theoretical basis for standardizing operation for avoiding light in clinical medication.Methods:First, a standard curve was created and methodological studies were conducted. Then, intravenous drip solutions were prepared using clinically prescribed methods. Then, the absorbance of the solution of cinepazide maleate injection under outdoor light, light emitting diode (LED) light, indoor light, artificial sunlight with a color temperature of 6 500 K (D65), and under a shading condition for 1, 2, 3, and 7 hours were brought into the standard curve to obtain the concentrations, and the change of the concentrations was studied.Results:There was no light decomposition phenomenon of cinepazide maleate injection in the shading group (control group) within 7 hours. In the other four experimental groups, there was no obvious light decomposition phenomenon of cinepazide maleate injection in the LED light group. Compared with the shading group, cinepazide maleate injection in the indoor, outdoor, and D65 groups began to exhibit a light decomposition phenomenon at 1 hour ( F = 44 840.44, P < 0.001). At 2 hours, cinepazide maleate content in the outdoor and D65 groups began to decrease significantly compared with the shading group ( F = 15 459.12, P < 0.001). At 7 hours, cinepazide maleate exhibited significantly greater light decomposition in the outdoor group [(29.84 ± 0.43) L·mol -1·cm -1] and D65 group [(21.01 ± 0.51) L·mol -1·cm -1] than the shading group [(101.65 ± 1.5) L·mol -1·cm -1] ( F = 63 106.32, P < 0.001). There was no significant difference in cinepazide maleate content between LED and shading groups ( P > 0.05). Conclusion:Cinepazide maleate injection is stable under the common LED light source and therefore it is not necessary to take lightproof operation. Cinepazide maleate injection is unstable under indoor, outdoor, and D65 light sources, and it is necessary to take lightproof operation.

ABSTRACT

Objective:To evaluate the effect of different doses of compound sodium chloride injection combined with norepinephrine on prevention of hypotension after lumbar anesthesia in the patients undergoing caesarean section.Methods:A total of 150 patients with a singleton fetus, aged 18-45 yr, at ≥37 weeks of gestation, of American Society of Anesthesiologists Physical Status classification Ⅰ or Ⅱ, with height ≥150 cm, weighing ≤100 kg, with body mass index < 40 kg/m 2, scheduled for elective caesarean section under lumbar anesthesia, were divided into 3 groups ( n=50 each) by the random number table method: compound sodium chloride injection 4, 8 and 12 ml·kg -1·h -1 groups (group A, group B, group C). Compound sodium chloride injection 4 ml/kg was intravenously injected for liquid preload before lumbar anesthesia, and 0.5% hyperbaric bupivacaine 12.5 mg was injected to the subarachnoid space for lumbar anesthesia. Norepinephrine was intravenously injected at a dose of 6 μg immediately after intrathecal injection, followed by an infusion of 0.05 μg·kg -1·min -1, and infusion was stopped at 5 min after delivery. Compound sodium chloride injection was intravenously infused simultaneously at a rate of 4, 8 and 12 ml·kg -1·h -1 in A, B and C groups, respectively. The maximum diameter of inferior vena cava (IVCmax) and the minimum diameter of inferior vena cava (IVCmin) were measured by ultrasound, and inferior vena cava collapse index (IVC-CI) was calculated at 1 min before fluid preload (T 1), immediately after fluid preload (T 2), at 5 min after anesthesia (T 3), at 5 min after fetal delivery (T 4) and immediately before leaving the operating room (T 5). The incidence of intraoperative adverse events (hypotension, severe hypotension, bradycardia, hypertension, nausea, and vomiting) and neonatal outcomes (umbilical artery blood gas index and Apgar score at 1 and 5 min after birth) were recorded. Results:Compared with group A, IVCmin was significantly increased and IVC-CI was decreased at T 5 in group B, and IVCmin and IVCmax were significantly increased and IVC-CI was decreased at T 5 in group C ( P<0.05). There was no significant difference in IVCmax, IVCmin and IVC-CI at each time point between group B and group C ( P>0.05). There was no significant difference in the incidence of hypotension, severe hypotension, bradycardia, hypertension, nausea and vomiting among the three groups ( P>0.05). There was no significant difference in the results of blood gas analysis of the umbilical artery and Apgar score at each time point after birth among the three groups ( P>0.05). Conclusions:Compound sodium chloride injection 4, 8 and 12 ml·kg -1·h -1 combined with norepinephrine can effectively prevent the occurrence of hypotension after lumbar anesthesia in the patients undergoing caesarean section without increasing maternal and infant adverse events, and the effect of 8 and 12 ml·kg -1·h -1 for volume supplementation is better than that of 4 ml·kg -1·h -1.

ABSTRACT

Objective:To investigate the effects of normal iodized salt diet on urinary iodine concentration and iodine uptake rate in patients with differentiated thyroid cancer (DTC) before 131I treatment. Methods:A prospective study was conducted on patients with DTC (59 male patients and 130 female patients, age (43.7±12.2) years) who received 131I treatment for the first time after total thyroidectomy in the First Affiliated Hospital of Zhengzhou University between January 2021 and April 2022. Patients were divided into normal iodized salt diet group and limited iodized salt diet group according to whether iodized salt diet was administered 4 weeks before 131I treatment. The age, gender, urinary iodine concentration, iodine uptake rate and tumor risk stratification of the two groups were compared by independent-sample t test or χ2 test. In addition, according to the concentration of urinary iodine, patients were divided into group a1 (urinary iodine <200 μg/L) and group a2 (urinary iodine ≥200 μg/L). Logistic regression analysis was used to analyze the factors affecting urinary iodine concentration. Results:The urinary iodine concentration of normal iodized salt diet group was not significantly different from that of non-iodized salt diet group ((140.53±76.66) vs (121.74±74.64) μg/L; t=1.67, P=0.489). The iodine uptake rates at 2 h, 4 h and 24 h in the 2 groups were (3.77±1.06)% vs (3.42±0.97)%, (3.33±1.07)% vs (3.21±1.15)%, (2.90±2.60)% vs (3.23±2.94)%, respectively ( t values: 2.33, 0.68, -0.81, all P>0.05). There were no significant differences in age ( t=0.56, P=0.889), gender ( χ2=1.33, P=0.250) and tumor risk stratification ( χ2=0.14, P=0.709) between the two groups. Logistic regression analysis showed that tumor risk stratification was associated with urinary iodine concentration (odds ratio ( OR)=3.914, 95% CI: 1.505-10.176; P=0.005). Conclusion:Normal iodized salt diet may have no effect on urinary iodine concentration and iodine uptake rate of patients with DTC before 131I treatment.

ABSTRACT

Introducción: La reducción del consumo de sal se ha identificado como una de las intervenciones prioritarias para prevenir las enfermedades no transmisibles a nivel mundial. Por ello, se recomienda que uno de los tres pilares para reducir su ingesta es reconocer el nivel de conocimientos, actitudes y prácticas relacionadas con el consumo de sal (CAP-sal). Objetivo: Determinar el nivel de CAP-sal y los factores asociados a estos en la población peruana. Materiales y métodos: Estudio transversal analítico realizado mediante encuesta virtual en población adulta peruana. Resultados: Se trabajó con una muestra de 918 sujetos. La proporción de participantes con conocimiento, actitudes y prácticas suficientes, fue del 54,58 %; 50,22 % y 40,31 %, respectivamente. Las variables que aumentan la probabilidad de tener un nivel suficiente de CAP-sal fueron el sexo femenino, la presencia de obesidad e HTA. Mientras que los antecedentes familiares y área de residencia lo fueron únicamente para conocimientos, el grado de instrucción para conocimientos y prácticas, y la actividad física tanto para actitudes como prácticas. Conclusiones: Tanto el nivel de conocimiento y actitudes fueron suficientes en la mitad de la muestra, pero las prácticas se encuentran por debajo de esta. Existen brechas que varían según las características sociodemográficas, como la edad, sexo, antecedentes de HTA, obesidad y realización de actividad física. Resultados que podrían apoyar la inclusión de la promoción de una nutrición saludable en la población peruana(AU)


Introduction: Reducing salt intake has been identified as one of the priority interventions to prevent non-communicable diseases worldwide. For this reason, it is recommended that one of the three pillars to reduce its intake is to recognize the level of Knowledge, Attitudes and Practices related to salt consumption (CAP-salt, in Spanish). Objective: To determine the level of CAP-salt and the factors associated with these in the Peruvian population. Materials and Methods: Cross-sectional analytical study carried out through a virtual survey of the Peruvian population. Results: We studied 918 subjects. The proportion of patients with sufficient knowledge, attitudes and practices was 54.58 %; 50.22 % and 40.31 %, respectively. The factors that increase the probability of having sufficient CAPsalt were female sex, the presence of obesity and hypertension. While family history and area of residence were only for knowledge, the degree of education for knowledge and practices, and physical activity for both attitudes and practices. Conclusions: The level of knowledge and sufficient attitudes are present in half of the population, but the practices are below this. In turn, several gaps vary according to sociodemographic characteristics, such as age, sex, history of hypertension, obesity, and physical activity, which could be the target of the new population awareness goals(AU)

Abstract Background Cardiovascular diseases are the major cause of hospitalization. Dietary salt restriction is indicated as part of clinical treatment, however, it is not always well accepted by the patients, resulting in low food intake and malnutrition. Objective To compare acceptance of a low-sodium diet cooked with salt with a standard low-sodium diet in cardiac inpatients. Methods A randomized controlled crossover trial in patients with low-sodium diet prescriptions (Clinical Trials NCT03481322). Patients were given a control standard low sodium diet (cooked without salt; salt [2g per meal] added by the patient at the time of consumption) on one day and on the next day patients were given the intervention diet - a low sodium diet cooked with salt (2 grams of salt, divided between preparations). Dietary acceptance was evaluated by weighing leftover food and calculating intake. A questionnaire was used to verify reasons that influenced acceptance. For data analysis, parametric data are presented as mean and standard deviation, Student's t test was used to compare means, with significance defined as p<0.05. Results Sixty-four patients were evaluated, with a mean age of 66 ± 11.3 years; 64% were male. There were no differences in percentage acceptance between the standard low-sodium diet and the low-sodium diet cooked with salt at lunch (p= 0.876) or at dinner (p= 0.255). Around 80% of what was offered at each meal was consumed by the patients, with no significant difference between groups. Conclusions The low-sodium diet cooked with salt was well accepted, but there was no difference when compared with the standard low-sodium diet, which also had adequate acceptance.

Introdução: O sal rosa do Himalaia tem se destacado no mercado brasileiro por sua coloração atrativa, além dos apelos ao consumidor que o destacam como um produto "mais natural" e com grande diversidade de minerais por ser um sal de rocha. O teor de iodo no sal rosa consumido no país ainda é pouco discutido, mas vital para manter sob controle os distúrbios por deficiência de iodo (DDI).  Objetivo: Avaliar amostras de sal rosa do Himalaia quanto aos teores de iodo, à presença de corantes e à rotulagem. Método: Foram analisadas 71 amostras em 13 cidades do estado de São Paulo para determinação de iodo adicionado na forma de iodato, prova qualitativa para corantes artificiais e avaliação da rotulagem com base na legislação brasileira. Resultados: Um elevado índice de  insatisfatoriedade dos teores de iodo foi encontrado (56%) e um percentual ainda maior nos sais comercializados a granel (74%). Verificou-se que, dentre as amostras insatisfatórias, o maior percentual de inadequação (28%) foi a ausência de iodo, colocando a população consumidora deste produto em risco para as DDI. A irregularidade de rotulagem mais encontrada foi a ausência da declaração da adição de iodo (46%), com contradições entre a declaração no rótulo e a efetiva presença avaliada analiticamente. Foram verificadas expressões de qualidade não previstas (27%) ou superlativas (14%), assim como alegações não comprovadas por estudos científicos, como a redução de 60% de sódio e a presença de 84 minerais. Nenhuma amostra apresentou adição de corante. Conclusões: O sal rosa do Himalaia analisado apresentou irregularidades importantes para a saúde da população, em especial quanto ao teor de iodo, mas também não conformidades de rotulagem que comprometem o acesso à informação correta sobre o produto.


Introduction: Himalayan pink salt has stood out in the Brazilian market for its attractive colors, in addition to appeals to the consumer that highlight it as a "more natural" product and with a great diversity of minerals as it is a rock salt. The iodine content in pink salt consumed in the country is still little discussed, but vital to keep Iodine Deficiency Disorders (DDIs) under control. Objective: Evaluate Himalayan pink salt samples for iodine contents, presence of artificial colorants and labeling. Method: Seventy-one samples from thirteen cities of the São Paulo State were analyzed for determination of iodine added as iodate, qualitative testing colorants, and labeling evaluation based on Brazilian legislation. Results: A high rate of unsatisfactory iodine content was found (56%), even higher in salts sold in bulk (74%). It was found that among the unsatisfactory samples, the highest percentage of inadequacy (28%) was the absence of iodine, exposing the consumers of this product at risk for DDIs. The most frequent labeling irregularity was the absence of iodine declaration (46%), with contradictions between label declaration and effective presence evaluated analytically. Unforeseen (27%) or superlative (14%) quality expressions were verified, as well as claims not supported by scientific studies, such as 60% reduction in sodium and presence of 84 minerals. No sample showed colorants addition. Conclusions: Himalayan pink salt samples analyzed showed important irregularities for health of the population, especially regarding the iodine content, but also labeling inaccuracies that compromise access to correct information about the product.

Resumo Fundamento O sal do Himalaia (SH) tornou-se uma alternativa popular para o sal de mesa (SM) devido às suas alegações de benefícios à saúde, principalmente para indivíduos com hipertensão arterial. Porém, apesar do aumento do consumo de SH, ainda faltam evidências clínicas que sustentem a recomendação de seu consumo por profissionais de saúde. Objetivo Este estudo teve como objetivo comparar o impacto da ingestão de SH e SM sobre a pressão arterial sistólica (PAS), pressão arterial diastólica (PAD) e concentração de sódio urinário em indivíduos com PA. Métodos Este estudo recrutou 17 pacientes do sexo feminino com hipertensão arterial que comiam fora de casa no máximo uma vez por semana. Os participantes foram divididos aleatoriamente em dois grupos, para receber e consumir SH ou SM. Antes e depois de cada intervenção, os participantes tiveram sua pressão arterial medida e urina coletada para análise mineral. Um valor de p <0,05 foi considerado estatisticamente significativo. Resultados Não houve diferenças estatisticamente significativas antes e depois da intervenção SH para PAD (70 mmHg vs. 68,5 mmHg; p = 0,977), PAS (118,5 mmHg vs. 117,5 mmHg; p = 0,932) e concentração urinária de sódio (151 mEq / 24h vs. 159 mEq / 24; p = 0,875). Além disso, a análise entre os grupos não mostrou diferenças significativas após a intervenção em relação a PAS (117 mmHg vs 119 mmHg; p = 0,908), PAD (68,5 mmHg vs 71 mmHg; p = 0,645) ou concentração urinária de sódio (159 mEq / 24h vs 155 mEq / 24h; p = 0,734). Conclusão Este estudo sugere que não há diferenças significativas no impacto do consumo de SH em relação ao SM na PA e concentração urinária de sódio em indivíduos com hipertensão arterial.


Abstract Background The Himalayan salt (HS) has become a popular alternative for the traditional table salt (TS) due to its health benefit claims, particularly for individuals with arterial hypertension. However, despite the increase in HS consumption, there is still a lack of clinical evidence to support a recommendation for its consumption by health professionals. Objective This cross-over study aimed to compare the impact of HS and TS intake on systolic blood pressure (SBP) and diastolic blood pressure (DBP), and urinary sodium concentration in individuals with arterial hypertension. Methods This study recruited 17 female patients with arterial hypertension who ate out no more than once a week. Participants were randomized into two groups, to receive and consume either HS or TS. Before and after each intervention, participants had their blood pressure measured and urine collected for mineral analysis. A p-value < 0.05 was considered statistically significant. Results There were no statistically significant differences before and after the HS intervention for DBP (70mmHg vs. 68.5mmHg; p=0.977), SBP (118.5 mmHg vs. 117.5 mmHg; p= 0.932) and sodium urinary concentration (151 mEq/24h vs. 159 mEq/24; p=0.875). Moreover, the between-group analysis showed no significant differences after the intervention regarding SBP (117mmHg vs 119 mmHg; p=0.908), DBP (68.5 mmHg vs. 71mmHg; p= 0,645) or sodium urinary concentration (159 mEq/24h vs. 155 mEq/24h; p=0.734). Conclusion This study suggests that there are no significant differences on the impact of HS consumption compared to TS on blood pressure and sodium urinary concentration in individuals with arterial hypertension.

RESUMEN La deficiencia de yodo (DI) causa daño a través de todos los ciclos de la vida, la vulnerabilidad es mayor durante la gestación y la infancia. La yodación universal de la sal (IUS) para consumo humano es la estrategia más costo-efectiva y sostenible para su control. Perú ha logrado la eliminación sostenida de los desórdenes por deficiencia de yodo (DDI) desde 1994. Objetivo: Determinar la efectividad del programa nacional de control de los DDI y la estrategia IUS para satisfacer el mayor requerimiento de yodo y asegurar la nutrición óptima de yodo de las mujeres embarazadas de la sierra, una región con severa deficiencia natural de yodo. Material y Métodos: El estudio ha incluido a 489 mujeres embarazadas de la sierra, seleccionadas entre las asistentes a control pre natal en los centros asistenciales. En cada sujeto se verificó el consumo de sal yodada y se colectó una muestra casual de orina para el análisis de la concentración de yodo y creatinina. Resultados: Según la encuesta de admisión el 99.6 % de los hogares consumen sal yodada. La mediana global de la concentración urinaria de yodo (CUI) 209 µg/L está dentro del rango adecuado para gestantes y demuestra un estado nutricional de yodo normal. La concentración de creatinina en la orina es normal. Conclusión: Estos resultados confirman la eficiencia y el éxito del programa nacional para la eliminación sostenida de los DDI, garantizando la nutrición normal de yodo durante la gestación y, por lo tanto, previniendo el riesgo de daño cerebral de los recién nacidos cada año en la sierra.


ABSTRACT Iodine is an essential element for synthesizing thyroid hormones, it is also essential for cell metabolism and tissue development, especially in the brain. Iodine requirements are higher during pregnancy and lactation. Iodine deficiency (ID) is a widespread condition all over the world; it is frequent in Peruvian highlands and rainforest. ID causes damage in all life periods, and vulnerability for this is greater during pregnancy and infancy. Universal salt iodination (USI) for human use is the most cost-effective and sustainable strategy for controlling ID. Peru has achieved the sustained elimination of iodine deficiency disorders (IDD) since 1994. Objective. To determine the effectiveness of the national program for controlling IDDs and the USI strategy for complying with the increased iodine requirement and to assure optimal iodine nutrition in pregnant women from the Peruvian highlands, a region with severe natural iodine deficiency. Material and Methods. The study included 489 pregnant women from the highlands, who were selected from those attending prenatal assessment in healthcare centers. Iodinated salty consumption was verified in each subject and a casual urine sample was collected for measuring iodine and creatinine concentration. Results. According to the admission survey, 99.6% of household use iodinated salt. The overall mean of iodine urine concentration (IUC) was 209 µg/L, which is well within the adequate range for pregnant women, and it shows a normal iodine nutrition status. The creatinine urinary concentration was normal. Conclusion. These results confirm the efficiency and success of the national program for the sustained elimination of IDDs, assuring normal iodine nutritional supply during pregnancy; and, therefore, preventing the risk for brain damage in newborns every year in the highlands.

ABSTRACT

A presente pesquisa teve por objetivo analisar os efeitos da substituição do sal de cozinha por sal de ervas sobre os valores de pressão arterial e excreção de sódio urinário de pessoas com hipertensão arterial. Trata-se de um estudo de abordagem quantitativa e delineamento quasi-experimental, tipo "cross-over", com amostra de hipertensos cadastrados em três unidades públicas de saúde de um município do estado de Mato Grosso. A coleta de dados foi realizada no período de maio de 2020 a maio de 2022. Por meio de entrevista, foi aplicado o questionário para identificação das variáveis sociodemográficas e o "Questionário de Adesão a Medicamentos - Qualiaids" (QAM-Q). Posteriormente, foi realizada a avaliação antropométrica e a medida casual da pressão arterial. A pesquisa foi dividida em três etapas: Etapa 1 - os participantes permaneceram em uso habitual do sal de cozinha, durante sete dias; Etapa 2 - os participantes fizeram uso exclusivo de cinco gramas de sal por dia, durante 10 dias; Etapa 3 - os participantes usaram exclusivamente o sal de ervas para o preparo das refeições, durante 10 dias. Em todas as etapas, foi realizada a Monitorização Residencial da Pressão Arterial (MRPA) e a dosagem de sódio urinário. As análises descritivas foram realizadas por meio do pacote estatístico IBM Statistical Package for Social Science - SPSS®, versão 25.0. O teste ANOVA foi utilizado para comparações entre as médias pareadas. Os resultados foram expressos como valores médios, desvio padrão (dp), mínimo, máximo e mediana. Para diferença estatística foi considerado p<0,05. Participaram do estudo 24 indivíduos, com idade média de 63,2 ± 10 anos, a maioria mulheres (54,2%), que convivem com companheiro (70,8%), de cor da pele autodeclarada branca (54,2%), com média de 7,6 anos de estudo e aposentados (45,8%). Em relação às variáveis clínicas, houve predomínio de participantes com pressão arterial ótima (45,8%), na categoria sobrepeso (45,8%), com risco cardiovascular elevado segundo relação cintura-estatura (87,5%) e risco muito elevado de acordo com a medida da circunferência da cintura (54,1%); 62,5% se mostraram aderentes ao tratamento. Na comparação entre as três etapas, observou-se redução significante nos valores de Pressão Arterial Sistólica (p=0,003) e Pressão Arterial Diastólica (p=0,001), sem diferença nas variáveis consumo de sal (p=0,66) e excreção de sódio urinário (p=0,66). Ainda que o uso do sal de ervas seja uma estratégia não medicamentosa interessante e inovadora para o controle da pressão arterial, os participantes não aderiram de forma expressiva à intervenção proposta. De qualquer forma, salienta-se que a monitorização do consumo de sódio é primordial para a avaliação do uso do sal pela população, tendo em vista a implementação de medidas visando a redução da pressão arterial e a prevenção de eventos cardiovasculares


The present research aimed to analyze the effects of replacing table salt with herbal salt on blood pressure values and urinary sodium excretion in people with hypertension. This study has a quantitative approach and quasi-experimental design, "cross-over" type, with a sample of hypertensive patients registered in three public health units in a municipality in the state of Mato Grosso. Data collection was carried out from May 2020 to May 2022. Through an interview, the questionnaire to identify sociodemographic variables and the "Medication Adherence Questionnaire - Qualiaids" (QAM-Q) were applied. Subsequently, anthropometric assessment and casual blood pressure measurements were performed. The research was divided into three stages: Stage 1 - the participants kept the usual use of table salt for seven days; Stage 2 - participants made exclusive use of five grams of salt per day for ten days; Stage 3 - participants used only herbal salt to prepare meals for 10 days. Residential blood pressure monitoring and urinary sodium measurement were performed at all stages. Descriptive analyzes were performed using the IBM Statistical Package for Social Science - SPSS® program, version 25.0. The One Way test was used for comparisons between the paired mean values. Results were expressed as mean values, standard deviation (sd), minimum, maximum, and median. For statistical difference, p<0.05 was considered. Participated in the research 24 individuals, with a mean age of 63.2 ± 10 years, most of them was women (54.2%), who live with a partner (70.8%), self-declared white skin color (54.2%), with an average of 7.6 years of formal education and retired (45.8%). Regarding clinical variables, it was observed a predominance of participants with optimal blood pressure (45.8%), in the overweight category (45.8%), with high cardiovascular risk according to the waist-to-height ratio (87.5%), and very high risk according to waist circumference measurement (54.1%), 62.5% were adherent to the treatment. When comparing the three stages, there was a significant reduction in the values of systolic blood pressure (p=0.003) and diastolic blood pressure (p=0.001), with no difference in the variables salt consumption (p=0.66) and urinary sodium excretion (p=0.66). Although the use of herbal salt is an interesting and innovative non-drug strategy for blood pressure control, participants did not significantly adhere to the proposed intervention. Nevertheless, it should be noted that the monitoring of sodium consumption is essential for the evaluation of the use of salt by the population and to implement measures aimed at reducing blood pressure and preventing cardiovascular events

ABSTRACT

Objective:To investigate the effects of intraoperative irrigation with hypothermic 3% sodium chloride solution on surgery effect and post-tonsillectomy pain in adults.Methods:A total of 120 adult patients with the plasma tonsillectomy from January to December 2020 in the First Affiliated Hospital of Soochow University were divided into observation group A, observation group B, observation group C and control group according to random number table method, with 30 cases in each group. During operation, observation group A used hypothermia 3% sodium chloride, observation group B used room temperature 3% sodium chloride, observation group C used hypothermia 0.9% sodium chloride, control group used room temperature 0.9% sodium chloride. The quality of surgical field quality, operation time, blood loss in operation, duration of catheter, the postoperative wound pain at 24, 48, 72 h after operation were compared among four groups.Results:The surgical field quality scores were (2.07 ± 0.87) points in the observation group A, significantly lower than in the observation group B (2.57 ± 0.73) points, observation group C (2.60 ± 0.56) points and control group (3.10 ± 0.66) points, the differences were statistically significant ( t=2.71, 2.89, 5.61, all P<0.01). Amounts of blood loss in operation were (9.13 ± 1.74), (10.03 ± 2.81), (10.50 ± 2.09) ml in the observation group A, observation group B and observation group C, significantly lower than in the control group (15.23 ± 3.64) ml, the differences were statistically significant ( t=8.85, 7.54, 6.87, all P<0.01). At 24 h after operation, the scores of Numerical Rating Scale(NRS) were (3.23 ± 0.77), (3.53 ± 0.63), (3.80 ± 0.89) points in the observation group A, observation group B and observation group C, significantly lower than in the control group (4.43 ± 1.17) points. At 48 h after operation, the scores of NRS were (2.37 ± 0.72) points in the observation group A, significantly lower than in the control group (2.93 ± 0.83) points, the differences were statistically significant ( t values were 2.77-5.25, all P<0.01).There was no significant difference in operation time, duration of catheter and wound pain 72 hours after operation among the four groups ( P>0.05). Conclusions:Intraoperative use of hypothermic 3% sodium chloride solution can reduce the amount of blood loss in operation in adult patients with plasma tonsillectomy, increase the clarity of visual field, and effectively reduce postoperative pain.

Resumo O objetivo do trabalho foi avaliar a adesão aos acordos voluntários de redução de sódio firmados entre indústrias de alimentos e o Ministério da Saúde e comparar as metas adotadas com o limite de sódio proposto no modelo de perfil nutricional da Organização Pan-Americana da Saúde (OPAS). Utilizaram-se informações de 1.553 alimentos de 32 categorias incluídas nos acordos e comercializados nas maiores redes de supermercados brasileiras em 2017. Foram calculadas as proporções de produtos com quantidade de sódio igual ou abaixo do limite proposto pelos acordos e pela OPAS. A concordância de classificação dos itens segundo os dois critérios foi avaliada com o coeficiente kappa de Cohen (k). Nossos resultados mostraram que 77,7% dos alimentos analisados estavam adequados segundo os acordos de redução de sódio, porém apenas 35,9%, segundo o modelo da OPAS. A concordância entre os dois critérios ao classificar um produto como adequado em relação ao conteúdo de sódio foi fraca (k = 0,199). Conclui-se que os acordos voluntários de redução de sódio são limitados em relação à abrangência e ao rigor das metas estabelecidas. A adoção de medidas voltadas a todos os produtos disponíveis, com metas mais restritivas e obrigatórias, deveria ser considerada no país.


Abstract The objective was to assess adherence to voluntary agreements for sodium reduction firmed between the food industries and the Ministry of Health in Brazil and to compare their targets with the limit proposed in the Pan American Health Organization (PAHO) nutritional profile model. We used data from 1.553 foods from 32 categories included in the agreements and sold in the largest Brazilian supermarket chains in 2017. The frequency of products with sodium equal or below the cut-offs proposed by the voluntary agreements and by PAHO was calculated. Classification concordance according to the two was evaluated with Cohen's kappa coefficient (k). Our results showed that 77.7% of products were adequate according to the voluntary agreements, and only 35.9% of them, according to the PAHO model. We identified a weak degree of concordance between both criteria in classifying a product as adequate about sodium content (k = 0.199). In conclusion, the voluntary agreements for sodium reduction are limited in their scope and rigor. The adoption of measures oriented for all products, with more restrictive and mandatory targets, should be considered in the country.

Abstract Objectives. To evaluate the association between knowledge, attitudes, and behavior (KAB) towards sodium use and sodium intake measured by 24-hour urinary collection in an adult cohort from Uruguay (Genotype Phenotype and Environment of Hypertension Study, GEFA-HT-UY). Methods. In a cross-sectional study (n = 159), a single 24-hour urinary sample, participants' physical, biochemical and blood pressure measurements and questionnaire data were collected. The association between KAB and 24-hour urinary sodium excretion was assessed using general linear models. Results. Mean age of participants was 49.8±15.5 years, 67.9% were women, and mean 24-hour urinary sodium excretion was 3.6±1.7 g/day. Although 90.6% of participants exceeded the maximum recommended intake as indicated by urinary sodium excretion, more than half misperceived their actual intake, reporting consuming "the right amount." Almost three-quarters of the participants reported being concerned about the amount of sodium in their diet, but only 52.8% reported taking action to control it. Lack of procedural knowledge was observed. There was no association between KAB and sodium use and intake assessed by 24-hour urinary sodium excretion. Conclusions. The lack of association between KAB towards the use of sodium and intake measured by 24-hour urinary excretion reflects the need to support people with opportunities and motivations to reduce sodium consumption. Structural actions to promote an adequate food environment, such as the effective implementation of the front-of-package nutrition labeling in Uruguay, are positive steps.

Evaluar la relación entre conocimientos, actitudes y comportamientos en lo relativo a la ingesta de sodio medida por la recolección de orina de 24 horas en una cohorte de adultos en Uruguay (GEnotipo, Fenotipo y Ambiente de la HiperTensión Arterial en UruguaY, GEFA-HT-UY). Métodos. En un estudio transversal (n = 159), se obtuvo una muestra urinaria de 24 horas y los datos de un cuestionario y de las mediciones físicas, bioquímicas y de presión arterial de los participantes. Se evaluó la asociación entre conocimientos, actitudes y comportamientos y la excreción urinaria de sodio en 24 horas con modelos lineales generales. Resultados. La edad media de los participantes fue 49,8±15,5 años, 67,9% eran mujeres y la excreción urinaria media de sodio en 24 horas fue de 3,6±1,7 g/día. Aunque 90,6% de los participantes excedieron la ingesta máxima recomendada de acuerdo con la excreción urinaria de sodio, más de la mitad percibió su ingesta real incorrectamente al señalar que consumía "la cantidad correcta". Casi tres cuartas partes de los participantes manifestaron estar preocupados por la cantidad de sodio en su dieta, si bien solo 52,8% declaró haber tomado medidas para controlarla. Se observó una falta de conocimiento sobre los procedimientos. No se encontró una asociación entre conocimientos, actitudes y comportamientos y la ingesta de sodio evaluada mediante la excreción urinaria de sodio en 24 horas. Conclusiones. La falta de relación entre conocimientos, actitudes y comportamientos respecto del consumo de sodio y su ingesta medida por excreción urinaria en 24 horas refleja la necesidad de apoyar a las personas con oportunidades y motivaciones para reducir el consumo de sodio. Adoptar medidas estructurales que promuevan unas condiciones alimentarias adecuadas, como la aplicación efectiva del etiquetado nutricional frontal en Uruguay, constituye un paso en la dirección correcta.

ABSTRACT Objective. To present some resources developed as part of the technical support of the Pan American Health Organization (PAHO) to Member States to reduce population dietary sodium intake, and to discuss the main challenges and opportunities to accelerate action toward sodium intake reduction in the Americas. Methods. Sources of information include a mapping of salt reduction policies conducted in 2019, reports from working group meetings, interviews conducted in 2020 and 2021 in seven countries, and technical documents developed around the Updated PAHO Regional Sodium Reduction Targets. Results. These tools show that, despite progress, challenges to succeed in this agenda persist. Priority given to sodium reduction is low in most countries, with insufficient resource allocation. There is a lack of intersectoral coordinated action, and a systemic approach to food systems is commonly missing. Surveillance mechanisms of sodium intake are insufficient, and industry interference in policy processes is commonly identified, undermining policy progress and success. There are also important regional opportunities to address these challenges. These include common ground for future collaborations by updating, strengthening, and complementing these existing tools, and technical and financial support for data generation. Conclusions. PAHO is committed to continue to support countries in the process of promoting, implementing, and monitoring cost-effective sodium reduction interventions. One key policy priority in this agenda is the adoption of the Updated PAHO Regional Sodium Reduction Targets with a mandatory approach, together with the comprehensive and complementary implementation of other strategies. Strong political will and commitment of countries will be critical to translate goals into concrete achievements in the Americas.


RESUMEN Objetivo. Presentar algunos recursos elaborados como parte del apoyo técnico brindado por la Organización Panamericana de la Salud (OPS) a los Estados Miembros para reducir la ingesta de sodio en los alimentos a nivel de la población y abordar los principales desafíos y oportunidades para acelerar las medidas de reducción de la ingesta de sodio en la Región de las Américas. Métodos. Entre las fuentes de información se encontraron un mapeo de las políticas de reducción de la sal realizado en el 2019, varios informes de reuniones de grupos de trabajo, entrevistas realizadas en siete países entre el 2020 y el 2021, y documentos técnicos acerca de las metas regionales actualizadas de la OPS para la reducción del sodio. Resultados. Estas herramientas muestran que, a pesar de los avances, persisten los desafíos en el logro de esta agenda. La prioridad dada a la reducción del sodio en la mayoría de los países es baja y la asignación de recursos es insuficiente. No hay coordinación intersectorial y por lo general no se adopta un enfoque sistémico para los sistemas alimentarios. Los mecanismos de vigilancia de la ingesta de sodio son insuficientes y es común que haya interferencia de la industria en los procesos que siguen las políticas, lo que socava su progreso y éxito. Hay importantes oportunidades regionales para abordar estos desafíos, como un terreno común para futuras colaboraciones mediante la actualización, el fortalecimiento y la complementación de las herramientas existentes, y el apoyo técnico y financiero para la generación de datos. Conclusiones. La OPS mantiene su compromiso de seguir apoyando a los países en el proceso de promoción, ejecución y seguimiento de intervenciones costo-eficaces para la reducción del sodio. Una prioridad política clave en esta agenda es la adopción con carácter obligatorio de las metas regionales actualizadas de la OPS para la reducción del sodio, junto a la ejecución integral y complementaria de otras estrategias. Una fuerte voluntad política y el compromiso de los países serán fundamentales para traducir estos objetivos en logros concretos en la Región de las Américas.

As fontes de informação incluem um mapeamento das políticas de redução de sal realizado em 2019, relatórios de reuniões do grupo de trabalho, entrevistas realizadas em 2020 e 2021 em sete países e documentos técnicos relativos às Metas regionais atualizadas da OPAS para a redução do sódio. Resultados. Essas ferramentas mostram que, apesar dos progressos, os desafios para o êxito dessa agenda persistem. Na maioria dos países, a prioridade dada à redução do sódio é baixa, com alocação insuficiente de recursos. Não há ação coordenada intersetorial e, em geral, nem abordagem sistêmica dos sistemas alimentares. Os mecanismos de vigilância da ingestão de sódio são insuficientes e é comum haver interferência da indústria nos processos políticos, o que prejudica o avanço e o êxito das políticas. Há também importantes oportunidades regionais para enfrentar esses desafios, como um consenso para futuras colaborações por meio da atualização, do fortalecimento e da complementação das ferramentas existentes, além de apoio técnico e financeiro para a geração de dados. Conclusões. A OPAS está empenhada em continuar apoiando os países no processo de promoção, implementação e monitoramento de intervenções de redução do sódio com boa relação custo-benefício. Uma prioridade política decisiva nessa agenda é a adoção das Metas regionais atualizadas da OPAS para a redução do sódio, com aplicação obrigatória, associada à implementação ampla e complementar de outras estratégias. A firmeza de vontade e o compromisso político dos países será crucial para materializar os objetivos nas Américas.

ABSTRACT

RESUMEN Objetivo Identificar los desafíos que conlleva la implementación de la política mundial de yodación de sal para el consumo humano. Métodos Se realizó una revisión de la literatura publicada entre el año 2000 y 2021 en PubMed, con los términos MeSH yodo, política pública, cloruro de sodio y los términos complementarios yodo/deficiencia, exceso. Se obtuvieron 141 artículos y se revisaron 50 aplicando como criterio de inclusión el abordaje de la implementación de la política de yodación. Resultados Se identificaron siete desafíos: sostenibilidad de la política, eliminar la brecha entre la explotación y la comercialización de la sal, prevenir la fortificación indiscriminada de los alimentos, promover la educación sobre la ingesta de sal yodada, controlar la ingesta excesiva de yodo, equilibrar la ingesta óptima de yodo con la reducción del consumo de cloruro de sodio y producir suficiente información oficial sobre la política. Conclusión La yodación universal de la sal es una política que requiere mayor sensibilidad a las situaciones locales de cada país para mitigar efectivamente el problema de salud pública de los desórdenes por deficiencia y exceso en la ingesta de yodo alrededor del mundo.


ABSTRACT Objective To identify the challenges involved in the implementation of the global policy on salt iodization for human consumption. Methods A review of the literature published between 2000 and 2021 in PubMed was carried out with the MeSH terms iodine, public policy, sodium chloride and supplementary terms iodine/deficiency, excess. 141 articles were obtained and 50 were reviewed, applying the iodization policy implementation approach as an inclusion criterion. Results Seven challenges were identified: sustainability of the policy, closing the gap between exploitation and commercialization of salt, preventing indiscriminate fortification of food, promoting education on iodized salt intake, controlling excessive iodine intake, balance optimal iodine intake with reduced sodium chloride intake and produce sufficient official policy information. Conclusion The universal iodization of salt is a policy that requires greater sensitivity to the local situations of each country to effectively mitigate the public health problem of disorders due to deficiency and excess intake of iodine around the world.

ABSTRACT

Objetivo: construir e validar instrumento para medir a intenção comportamental de reduzir o consumo de sal na dieta em pessoas com insuficiência cardíaca. Materiais e métodos: estudo metodológico, fundamentado na Theory of Planned Behavior, desenvolvido em três etapas: construção do instrumento, validação de conteúdo com juízes especialistas e validaçãosemântica com 30 pessoas da população- alvo, de março a setembro de 2018, no município de João Pessoa, Brasil. Resultados: participaram da validação de conteúdo seis juízes especialistas. O instrumento conta com 30 itens distribuídos nos construtos atitude, norma subjetiva e controle comportamental percebido. O índice de validade de conteúdo mostrouse satisfatório (≥ 80 %). Na validação semântica, o instrumento foi considerado claro e compreensível. Conclusões: como produto, dispõe-se de instrumento válido quanto ao seu conteúdo e compreensão, podendo ser aplicado em estudo-piloto para avaliar sua qualidade psicométrica em medir o comportamento de interesse no contexto da insuficiência cardíaca.


Objetivo: construir y validar un instrumento para medir la intención de reducir el consumo de sal por parte de personas con insuficiencia cardíaca. Materiales y métodos: estudio metodológico basado en la teoría del comportamiento planeado, el cual se desarrolló en tres etapas: construcción del instrumento, validación de contenido por juicio de expertos y validación semántica con 30 personas de la población objetivo, entre marzo y septiembre de 2018, en el municipio de João Pessoa, Brasil. Resultados: seis jueces expertos participaron en la validación de contenido. El instrumento consta de 30 ítems distribuidos en los constructos actitud, norma subjetiva y control conductual percibido. El índice de validez de contenido fue satisfactorio (≥ 80 %). Tras la validación semántica, el instrumento fue considerado claro y comprensible. Conclusiones: como producto, existe un instrumento válido en cuanto a contenido y comprensión, el cual puede ser aplicado en un estudio piloto para evaluar su calidad psicométrica en la medición de la conducta de interés en el contexto de la insuficiencia cardíaca.


Objective: To build and validate an instrument to measure the behavioral intention to reduce the consumption of dietary salt in people with heart failure. Materials and methods: Methodological study based on the theory of planned behavior and developed in three stages: construction of the instrument, content validation with expert judges, and semantic validation with 30 people from the target population, from March to September 2018, in the municipality of João Pessoa, Brazil. Results: Six expert judges participated in content validation. The instrument has 30 items distributed in the constructs attitude, subjective norm, and perceived behavioral control. The content validity index was satisfactory (≥ 80 %), and the semantic validation of the instrument was considered clear and understandable. Conclusions: As a product, the instrument proved to be valid in terms of content and understanding. This tool can be applied in a pilot study to assess its psychometric quality for measuring the behavior of interest in the context of heart failure.

Background: Excessive sodium intake is associated with increased cardiovascular morbidity and mortality. Daily sodium intake is usually inferred from sodium excretion in a 24-hour urine collection, which is cumbersome and prone to errors. Different formulas have attempted to estimate 24-hour urinary sodium from a spot urine sample. Unfortunately, their concordances are insufficient and have not been tested in our population. Aim: To develop an equation to predict 24-hour urine sodium from parameters in plasma and spot urine samples. To validate the equation and compare it with other formulas in Chilean population. Material and Methods: Analysis of 24-hour urine collections, plasma sample and spot urine sample from 174 adult outpatients (81% females) with an estimated glomerular filtration rate (eGFR) ≥ 60 mL/min/1.73m2. These were collected between 2015 and 2019 using standardized methods and educating patients about the correct method to collect 24 h urine samples. In all these patients, creatinine and electrolytes were measured in plasma and urine. A new equation was developed using a multiple linear regression model. Results: Twenty-four-hour urine sodium excretion was significantly correlated with age, weight, height, eGFR, plasma osmolarity, urine electrolytes and parameters obtained from spot urine sample, among others. The new equation had a linear correlation with 24-hour natriuresis of 0.91 and the concordance was 0.9. The predictive capacity of the new equation was better than the existing formulas. Conclusions: We developed a formula to accurately predict daily natriuresis in the Chilean population.
